What the Immigration Physical Involves

The immigration physical is a standardized exam required by USCIS for all applicants seeking a green card (permanent residency) through adjustment of status. A designated civil surgeon must perform the exam and complete Form I-693, which is submitted with your immigration application.

During the exam, your civil surgeon reviews your medical history and immunization records, performs a physical and mental evaluation, checks your vaccination records against USCIS requirements, and orders any required testing. This includes a blood draw for syphilis screening and either a TB blood test (interferon-gamma release assay) or a tuberculin skin test. Required testing can also include a chest X-ray when indicated.

Vaccination Review and Administration

USCIS requires proof of a specific list of vaccinations. If your records show gaps, we can administer many of the required vaccines during your visit. Bring whatever vaccination records you have, even if they are incomplete. Our team will review what you need and work with you on a plan to get current.

On-Site Lab Work

Avita MD has on-site phlebotomy at all three locations, so your required blood work happens during the same visit. There is no need to schedule a separate appointment at an outside lab. This helps you get your completed I-693 form faster.

Form I-693 Completion

After your exam and lab results are in, your civil surgeon completes and signs Form I-693. The form is valid for two years from the date the civil surgeon signs it. We review the form with you before sealing it, so you understand every section. You receive the sealed envelope to submit with your USCIS application.

How to Prepare for Your Appointment

To make the most of your visit and avoid delays:

  • Bring a valid photo ID (passport, driver's license, or state ID)
  • Bring all available vaccination records from any country
  • Bring medical records for any chronic conditions or past surgeries
  • If you take medications, bring a list or the medication bottles
  • Wear short sleeves for blood draw and any vaccinations

Immigration physicals are not covered by health insurance. The examination fee includes the physical exam, Form I-693 completion, and required TB screening. Vaccinations are billed separately. Call us at (408) 495-5770 for current pricing information.

Frequently Asked Questions About Immigration Physicals

What is an immigration physical?

An immigration physical is a medical examination required by USCIS (U.S. Citizenship and Immigration Services) as part of the green card application process. It must be performed by a USCIS-designated civil surgeon. At Avita MD, our designated civil surgeons complete the I-693 form and ensure all required vaccinations and screenings are documented.

Who can perform an immigration physical?

Only physicians designated as civil surgeons by USCIS can complete and sign the immigration medical exam. Avita MD's USCIS-designated civil surgeons are Dr. Patricia Sitnitsky, M.D. and Dr. Jyotsna Singh Vincent, M.D. Every Form I-693 is completed and signed by Dr. Sitnitsky or Dr. Vincent, and immigration physicals are available at all three Avita MD offices in Los Altos, Mountain View, and Los Gatos.

Is Avita MD authorized to complete Form I-693?

Yes. Avita MD's USCIS-designated civil surgeons, Dr. Patricia Sitnitsky, M.D. and Dr. Jyotsna Singh Vincent, M.D., are authorized to perform the green card medical exam and complete Form I-693. Every I-693 is signed by Dr. Sitnitsky or Dr. Vincent. Our physician assistants may assist with parts of the exam under their direct supervision, which is the only way a civil surgeon's signature can be applied to your form.

What does the green card medical exam include?

The green card medical exam at Avita MD includes a review of your medical history and immunization records, a physical and mental evaluation, and required testing. Required testing can include a chest X-ray when indicated, along with screening for tuberculosis and syphilis. Your civil surgeon then completes and signs Form I-693 for your USCIS application.

What vaccinations are required for the immigration physical?

USCIS requires applicants to show proof of vaccination against mumps, measles, rubella, polio, tetanus, diphtheria, pertussis, hepatitis A, hepatitis B, rotavirus, meningococcal disease, varicella, pneumococcal disease, seasonal influenza (during flu season), and COVID-19. If you are missing any vaccinations, Avita MD can administer them during your visit or advise you on where to get them.

What should I bring to my immigration physical appointment?

Bring a valid government-issued photo ID (passport or driver's license), your vaccination records, and any medical records for previously diagnosed conditions. If you wear glasses or contacts, bring them. Avita MD also recommends bringing your I-797 approval notice if you have one.

How long does the immigration physical take?

The appointment itself takes about 30 to 45 minutes. If lab work or additional vaccinations are needed, allow extra time. Avita MD completes on-site blood draws and TB testing at all three locations, so you do not need a separate lab visit.
